ICD-9/10-CM

The ICD-9-CM code set is modeled after the International Classification of Diseases, which is used throughout the world and maintained by the World Health Organization (WHO).
The National Center for Health Statistics (NCHS), a part of the Centers for Disease Control (CDC), maintains: Volumes 1 and 2, and CMS, which maintains Volume 3.
The code set has five primary applications, see the image below:

ICD-9-CM updates called the addenda that take effect on October 1 and April 1 of every year. The October 1 changes are the major updates; the April 1 changes catch up on codes that were NOT included in the major changes. The major new, invalid, and revised codes are posted on the Mandated Code Set ICD-9-CM is the mandated code set for medical diagnoses and hospital inpatient services.

Billing Tip: Use Current Codes Compliant coding under HIPAA requires codes to be current as of the date of service. Do not report codes that are no longer in the code set. 

The Coding System:
1. Diseases and Injuries: Tabular List: Volume 1 
Is made up of seventeen chapters of disease descriptions and codes with two supplementary classifications and five appendixes. 
2. Diseases and Injuries: Alphabetic Index: Volume 2, which contains medical terms, is used first. After a code has been located in Volume 2 based on the diagnostic statement, it is verified in Volume 1 , known as the Tabular List .

3. Procedures: Tabular List and Alphabetic Index: Volume 3 
Covers inpatient procedures billed by hospitals. 

Providers offices and healthcare facilities use the Tabular List of Diseases and Index to Diseases (Volumes 1 and 2 of ICD-9-CM) to code diagnoses. The Index to Procedures and Tabular List of Procedures is used by hospitals to code inpatient procedures. 

Many publishers offer their own versions of ICD-9-CM and as a result, hospital (Volumes 1, 2, and 3) and outpatient (Volumes 1 and 2) editions of the coding manual are available. In addition to coding procedure easier, publishers often place the Index to Diseases (Volume 2) in front of Tabular List of Diseases (Volume 1).

ICD-9-CM Version - The Tabular List of Diseases (Volume 1) contains 17 chapters that classify diseases and injuries, two supplemental classifications, and appendices. The 17 chapters are organized as follows:

Appendix A: Morphology of Neoplasms (M codes) 
Appendix C: Classification of Drugs by American Hospital Formulary Service List Number and Their ICD-9-CM Equivalents.
Appendix D: Classification of Industrial Accidents According to Agency
Appendix E: List of Three-digit Categories
Note Appendix B: Glossary of Mental Disorders - was officially deleted October 1, 2004 not counted no more on appendices.

Note: ICD-9-CM Version is numeric codes, only V & E codes in I-9 version are alphanumeric; while ICD-10-CM version is alphanumeric codes.

ICD-10-CM Version: The Tabular List of Diseases (Volume 1) contains 21 chapters compared with 17 chapters and 2 supplemental classifications of V codes and E codes in ICD-9-CM tabular list of diseases. (ICD-10, created by WHO, has 22 chapters; ICD-10-CM excludes the chapter entitled Codes for special purposes, U-00-U99)

ICD-10-CM Special Attention!:
* In ICD-10-CM, "Factors Influencing Health Status and Contact with Health Services" are incorporated into tabular list of Chapter 21. It is no longer considered Supplementary Classification if you take a look the (Volume 1) Tabular List of ICD-9-CM!!!

* It remains to be seen whether ICD-10-CM will contain appendices similar to those in ICD-9-CM. The electronic files in posted at www.cms.hhs.gov do not include appendices for ICD-10-CM.

Example of E Code - External Causes of Injury
Incident:
Do you remember that plane crash that happened in San Francisco International Airport last July 26, 2013? There is a code for that, yes it is!
An E code  consists of four-character number (an E followed by three digits) and one digit after a decimal point. The Index to the External Causes (E Codes) is located separately in the Index to Diseases in Volume 2, after the Table of Drugs and Chemicals.

Coding Tip:
1. Review the note at the beginning of the E-Code tabular list before coding external causes of injuries and poisoning.
2. At the end of the Index to External Causes is a section entitled: "Fourth-Digit Subdivisions of the External Causes (E) Codes."
3. It maybe necessary to consult the Appendix E List of Three-Digit Categories for assistance in locating possible main terms in the E code index.
Coding Tip: Make sure that diagnoses, procedures an services listed on the encounter form or superbill are documented in the patient's medical record before reporting codes on the insurance claim.
